Graft stretching typically results from improper tunnel placement and increased strain associated with flexion or extension in case of a too far anterior or posterior placed femoral tunnel 2-5.
Most studies have reported that the ACL is composed of two bundles: AM and PL. 1,2,3,6 In many studies, the average length of the ACL is approximately 37 mm–41 mm7,8,9 The length of the ACL varies depending on the knee position.
The exact etiology of tunnel enlargement however remains unclear and is believed to be a multifactorial process including both mechanical and biological factors. Mechanical factors include graft tunnel-motion, especially in tunnel malposition; drill-related bone necrosis; and aggressive rehabilitation.
The average diameter of the ACL was 4.80 mm (3.1–8.3 mm), with a length of 3.8 cm (2.85–4.5 cm). The origin ranged from 9.7 mm to 15.4 mm. The average insertion on the tibia was 13.3 mm.
Heating parts of the partially torn ACL causes them to shrink. This tightens the ligament and allows it to better stabilize the joint. Thermal shrinkage is done during an arthroscopic procedure. A long, thin, lighted tube called an arthroscope is inserted into the knee joint and used to see and operate inside the knee.
Anterior cruciate ligament graft stretching can lead to the following conditions: chronic instability. chondral injury. meniscal tears.
1. Expected immediate post-operative ACL tunnel diameter should be around 10 mm regardless of graft selection. 2. Tunnel widening is generally cavitary, frequently maximal in the mid-zone of the tibial tunnel.

The mean size of the lateral femoral condyle was 62.2 mm (range: 48.1–74.7 mm), while the mean size of the medial femoral condyle was 55.7 mm (range: 43.2–67.4 mm).
