If we have your company classified as a single-unit business, you will only complete one MA-10000 survey or one NC-99007 survey. You will get a question at the end of your survey to provide how many establishments operated under the Employer Identification Number at the end of the year. Enter the number of establishments, and click the Save and Continue button. If 0 or 1, you will go to the Step 2 - View Errors/Warnings screen. If 2 or more, then that number of blank locations will appear on the Locations List screen so that you can provide data for each location. If we have your company classified as a multi-unit business, you may complete multiple surveys and/or have multiple locations listed on the NC-99001 survey. To add an image use the Insert menu in Crystal Reports: Open the form or report printing window in Sage 100. Click Designer button to open the form or report in Crystal Reports Designer. On the Insert menu, click Picture. Browse to the folder where the image is located.
Crystal Reports: Dynamic Images right click image. select Format Graphic select Picture tab. click the conditional-formula button (looks like x+2) set the formulas text to the name of the formula or parameter field that will contain the images URL. save the formula and click the OK button.
In Crystal Reports, open a report. Create a parameter, and set a List of Values that will correspond to the image to display on the report. Insert a static picture on the report, where you want the picture to display. Click on Save and Close.
