To attach a file or files to the list item, in the New item window, select Add attachments, and then select the file. Hold down the CTRL key when selecting files to attach more than one file. Select Open, and then select OK. If the Add attachments command is unavailable, your list doesnt support attachments.
Create a list on a classic SharePoint or a SharePoint Server 2019 site Select Settings. Select + New, and then select List. Enter a Name for the list, and optionally, a Description. Select Create. When your list opens, to add room for more types of information to the list, select + or + Add column.

Click on the Insert tab in the ribbon and then click on Link or from the dropdown, click From Sharepoint. A box will appear that shows your SharePoint Documents library. Click add (in Click to add new item) at the top to upload the PDF document.

In the content field select the question from your form where you asked the user to upload the file. Next, add an apply to each action and in the first field select Body from the Parse JSON section of the dynamic content box. Finally, inside the apply to each add a SharePoint add attachment action.
If you want to use the Attachments column in the SharePoint list, you can find the Attachment column in it by selecting the Show or hide columns option in the add column. The function of directly add attachment is not provided in Create item, you can add the action of Add Attachment.
