You can add an image to a question or an answer for multiple choice or checkbox questions. In Google Forms, open a form. Click a question or answer. Upload or choose an image.
Open the form that was sent. Find the Upload Photograph / Signature option in the file. Browse the photo from the location it was saved on your device. Check the format of the picture here.
In Microsoft Forms, open the form you want to edit. Select the question to which you want to add a picture. Select Insert media (picture icon) on the right side of the question. In the Insert media pane, select Image.
Allow people to attach files In Forms, open a form or quiz. and enter your request for information. and select File upload. If prompted, review the uploading information and click Continue.
