Each item on a form has a comment icon on the right side of the screen. So if you want to leave a comment about the subjects high temperature, you click the comment icon for that item. Add a comment. And click Submit to save the comment. Note that there is now an entry in the comment history. The Comment History drop down can be used to look back over previous comments if there are any. Click Return to go back to the form. To add a comment at the form level, click the comment icon in the top right header. Here you can enter a comment and, if applicable, select a reason why the form is not being completed. If you only enter a comment, it will only be applied at form level.
