hi im linda charlton today ill show you how to put a consent form in your survey im in my my account so lets get started so the consent page i already have it copied from my word and i would like to put it in question one and to do that were gonna change the question one two say survey type will be text graphic and were gonna click on where it says click click to write the question text and were going to click on the rich text editor were going to highlight the wording and were going to do ctrl v and then were gonna click away and we have the consent page added to your question so we now have your consent form added to your survey and now to make that block accessible highlight the block and go to next previous and make sure that the next button says next and hit save the consent page should be on its own block and only that in that block thanks for watching
