Additional information can be added to your SCR by your GP practice and is a summary of information about your medical history. It can include the following: Your long term health conditions such as asthma, diabetes, heart problems or rare medical conditions.
Log in to your NHS account, using your NHS App or a web browser. Select Linked profiles. Select the name of the person you want to act for.
Patients can ask to view or add information to their SCR by visiting their GP practice.
We'll update your NHS medical record with your new address. This usually takes 5 working days.
Accessing your record. If you have access to your GP health record in your NHS account, you'll be able to see your summary record using the NHS App or the NHS website. It contains information about your medicines and allergies.
A request for information from health (medical) records has to be made with the organisation that holds your health records – the data controller. For example, your GP practice, optician or dentist. For hospital health records, contact the records manager or patient services manager at the relevant hospital trust.
To see your records you will have to apply to the organisation that is responsible for them, for example: ​your GP practice manager. your dental surgery manager. the records manager at your hospital.
Please note that only data entries which are added to GP records after the switch on will be viewable. Data recorded before that date (i.e. previous medical history) will not be made available through the NHS App at this time.
Your Summary Care Record contains basic information about allergies and medications and any reactions that you have had to medication in the past. Some patients, including many with long term health conditions, have previously agreed to have Additional Information shared as part of their Summary Care Record.
A request for information from health records has to be made with the organisation that holds the health records – the data controller. This can be your GP, optician or dentist. You'll need to contact the organisation and ask them how to make a subject access request for your medical records.
