Contractions can be described by frequency, duration, strength (amplitude), uniformity, and shape. During normal labor, the amplitude of contractions increases from an average of 30 mm Hg in early labor to 50 mm Hg in later first stage and 50 to 80 mm Hg during the second stage.
How is your TOCO number measured? TOCO numbers are displayed by a unit called mmHg, which stands for millimeters of mercury, and its a measurement of pressure. When looking at your monitoring screens, the fetal heart rate is usually on the top left of the screen and the contractions/TOCO number to the right.
The most common risk factors causing fetal hypoxia are placental insufficiency, preclampsia, umbilical cord damage and maternal factors such as smoking, heart, kidney or lung dysfunction [3]. Prenatal hypoxia can be classified into three patterns: pre-placental; uteroplacental and post-placental.
What is a TOCO number? The pressure-sensitive contraction transducer, called a tocodynamometer or TOCO for short, records the pressure force produced by the contorting abdomen during uterine contractions.
When youre looking at the screen, the fetal heart rate is usually on the top and the contractions at the bottom. When the machine prints out graph paper, youll see the fetal heart rate to the left and the contractions to the right. Sometimes its easier to read printouts by looking at them sideways.
The fetal heart rate tracing shows EITHER of the following: Sinusoidal pattern OR absent variability with recurrent late decelerations, recurrent variable decelerations, or bradycardia.
Kingdom and Kaufmann classifies three categories for the origin of fetal hypoxia: 1) pre-placental (both mother and fetus are hypoxic), 2) utero-placental (mother is normal but placenta and fetus is hypoxic), 3) post-placental (only fetus is hypoxic).
Intrapartum cardiotocography traces were assigned to one of these four types of labor hypoxia: acute, subacute, gradually evolving and chronic hypoxia.
Common signs of asphyxia and hypoxia before delivery include: erratic fetal heart rate (usually measured on a fetal heart monitor); At birth, visible signs of fetal asphyxia and hypoxia include: blue or pale skin color; low heart rate; weak muscle tone; poor reflexes; weak cry; no cry; and. difficulty breathing.
The fetal brain sparing response to acute hypoxia is triggered by a carotid chemoreflex that leads to bradycardia and an increase in peripheral vasoconstriction.
