Dentists may offer in-office or at-home professional teeth whitening treatments. These methods use hydrogen peroxide or carbamide peroxide to break up stains and lift them from your teeth. Professional whitening works best on surface (extrinsic) stains.
Two methods exist for removing extrinsic stains: air-powder and rubber cup polishing. A fulcrum provides stability for the operator, but must facilitate movement of the wrist and forearm.

Extrinsic tooth stains are typically caused by tobacco use or by regularly drinking coffee and tea, wine or cola drinks. This type of tooth stain responds well to regular dental cleaning and brushing the teeth with whitening toothpaste.
Intrinsic Stains These stains are a little tougher to treat, but you may be able to remove them at home using a product with an active whitening ingredient like hydrogen peroxide or carbamide peroxide. A professional whitening treatment, though, might prove more effective.
Extrinsic stains occur on the external surface of the tooth and can be removed by self-care methods (such as brushing and flossing) and scaling and polishing (traditional or air polishing). Intrinsic stains occur on the internal surface of the tooth and cannot be removed by self-care methods.
