The term adaptation, as used in this chapter, refers to the deliberate modification of a question or questionnaire to create a new question or questionnaire. It is also referred as asking different questions (ADQ) in Questionnaire Design.
There are multiple advantages to adapting an existing instrument for a study, including cost savings, time savings, and the relatively fewer steps required for effective instrument adaptation compared to developing a new instrument [1].
An adaptive questionnaire is a hierarchically organized test instrument that employs contingency branching and automated question selection to focus subjective judgments on issues of maximal concern.
In general, adopting (using verbatim) is preferable to adapting for a few reasons. First, when the instrument is adopted, then the reliability and validity research studies that have been conducted on that instrument can be applied to your study, so you do not have to collect validity evidence.
Questionnaire development involves rigorous testing to ensure reliability and validity. Due to time and cost constraints of developing new questionnaires, researchers often adapt existing questionnaires to better fit the purpose of their study.
Questionnaire Validation in a Nutshell Generally speaking the first step in validating a survey is to establish face validity. The second step is to pilot test the survey on a subset of your intended population. After collecting pilot data, enter the responses into a spreadsheet and clean the data.

The term cross-cultural adaptation is used to encompass a process that looks at both language (trans- lation) and cultural adaptation issues in the process of preparing a questionnaire for use in another setting. Cross-cultural adaptations should be considered for several different scenarios.
The first is to take the instrument nearly verbatim, which is called adopting the instrument. On the other hand, you can docHubly alter the instrument, which is called adapting. In general, adopting (using verbatim) is preferable to adapting for a few reasons.
The wording of questions should never be changed. Shortened versions should only be used when they have been validated 5. Questions should never be added even if they are not going to be included in the analysis because they may alter how patients respond to other questions in the questionnaire.
