Overview. A heart attack or acute myocardial infarction (MI) occurs when one of the arteries that supplies the heart muscle becomes blocked.
Your health care provider will order this test if you have chest pain and other signs of a heart attack. The test is usually repeated two more times over the next 6 to 24 hours. Your provider may also order this test if you have angina that is getting worse, but no other signs of a heart attack.
The initial treatment for all ACS includes aspirin (300 mg) and heparin bolus and intravenous (IV) heparin infusion if there are no contraindications to the same. Antiplatelet therapy with ticagrelor or clopidogrel is also recommended.
Inhibition of platelet aggregation with aspirin and anticoagulation with unfractionated heparin can be considered the gold standard treatment of patients with acute coronary syndromes.
INITIAL ASSESSMENT All patients with chest pain (acute coronary syndrome [ACS]) should have an initial assessment (ie, electrocardiogram [ECG], history, physical examination) to rapidly confirm or exclude the diagnosis of STEMI and to identify other conditions that would change management.
Anticoagulant Drugs. Either a low molecular weight heparin (LMWH), unfractionated heparin, or bivalirudin is given routinely to patients with acute coronary syndrome unless contraindicated (eg, by active bleeding or planned use of streptokinase or anistreplase).
Initial therapy for ACS should focus on stabilizing the patients condition, relieving ischemic pain, and providing antithrombotic therapy to reduce myocardial damage and prevent further ischemia.
Initial management The management of ACS aims to provide supportive care and pain relief, and to prevent progression of cardiac injury. Pain relief should be offered as soon as possible with glyceryl trinitrate (sublingual or buccal). A loading dose of aspirin should be given as soon as possible.
