when you have heart failure the amount of blood being pumped through your heart and around your body is reduced your heart will adapt in three ways to try and increase your blood flow your heart beats faster to keep the blood moving through it it stretches to expand its size allowing it to hold more blood and it develops thicker muscle to help it pump more blood with each beat these changes in your heart help to increase blood flow around your body however over time these changes known as compensation put more stress on your heart and may actually worsen your heart failure

In congestive heart failure, peripheral adaptive mechanisms play a docHub and largely underestimated role. In acute heart failure, sympathetic-mediated peripheral vasoconstriction together with chronotropic and inotropic actions serves to maintain perfusion pressure and blood supply to the vital organs.
Direct CCFs are characterized by a direct connection between the internal carotid artery (ICA) and the cavernous sinus, whereas dural CCFs result from an indirect connection involving cavernous arterial branches and the cavernous sinus.
The most common findings include arterialization of the conjunctival veins, chemosis, proptosis, diplopia, bruit, retro-orbital headache, elevated intraocular pressure, and a decrease in vision. The classic triad of ocular symptoms seen in direct CCFs are less commonly seen in indirect CCFs.
CCFs can be classified based on the hemodynamic properties, the etiology, or the anatomy of the shunt. Hemodynamically, the fistulas can be classified as: Low flow fistulas, and. High flow fistulas.
Following this initial decline in pumping capacity of the heart, a variety of compensatory mechanisms are activated, including the adrenergic nervous system, the renin angiotensin system, and the cytokine system.
Main. A carotid-cavernous sinus fistula (CCF) is an abnormal communication between arteries and veins within the cavernous sinus. CCFs may be classified into four types: direct fistulas (Barrow type A) and dural, or indirect, fistulas (Barrow types B, C, and D).
Compensation in the body The bodys hormone and nervous systems try to make up for this. They increase blood pressure, hold on to salt (sodium) and water in the body, and increase the heart rate. These responses are the bodys attempt to compensate for the poor blood circulation and the backup of blood.
Symptoms. Direct CCF often occurs days or weeks after a closed head injury. Patients present with the classic triad of chemosis (red eye syndrome), pulsatile exophthalmos (abnormal protrusion of the eyeball), and ocular bruit (blood flow sounds coming from the eye).
Carotid cavernous fistulas can be diagnosed by imaging studies such as: CT scan. MRI scan. Angiogram.
A CCF can be due to a direct connection between the cavernous segment of the internal carotid artery and the cavernous sinus, or a communication between the cavernous sinus, and one or more meningeal branches of the internal carotid artery, external carotid artery or both.
