(1) The purchaser must retain Copy 3 of each executed triplicate DEA Form 222 and all copies of unaccepted or defective forms with each statement attached. This form is for new applicants. Any person who does not currently possess a DEA registration to conduct business with controlled substances in the following categories may access the application form.
