The Drug Enforcement Administration (DEA), Office of Diversion Control, will accept requests from distributors that require a large volume of Order Forms (DEA Form 222) with the pin feed tracking left on the form. Order Forms are used for the distribution of a Schedule I or II controlled substance.
To complete a Form 222: Complete Part 1. Name and Title. Signature of DEA Registrant. Date. Enter name, size, and quantity of controlled substance(s). Complete Part 2. Supplier DEA Number. Supplier address. Make a copy of the signed form before submitting your order form. Fill in Part 5 on copy once the order is received.
APPLICATION FOR REGISTRATION. Under the Controlled Substances Act.
Form 224 Practitioner; Mid-Level Practitioner; Pharmacy: Valid for three years with paid registration. Form 225 Researcher; Analytical Lab: Valid for one year with paid registration.
Under 21 CFR 1305.03, the completion of DEA Form 222 is required for each distribution of a schedule I or II controlled substance.
