Select Prepare Form from the Tools center or right-hand pane. Select an existing document to use as your form (for example, a Word, Excel, InDesign, or PDF file) and select Start. If Form field auto detection is ON, form fields are automatically detected in the PDF file. Double check placement of the form fields.
From the hamburger menu, select Preferences (Windows), or select Acrobat / Acrobat Reader Preferences (macOS). Select Forms on the left.
From the top toolbar, choose Edit Prepare a form. In the Prepare a form tool, select Start from blank page Create form. The form components are displayed on the left. To add text, select the Text option and type the desired text or information wherever you want to in your form.
Prepare Form tool is only available with the desktop application. There are couple of feature and services of Acrobat that can be accessed through DC web interface, but not all of them. Web interface has certain limitations. If you would like to prepare form, then use Acrobat DC desktop app.
You can send or distribute your PDF form right from within Acrobat. Open the form and click Distribute. Choose how you want to collect the responses - in your email Inbox or on a server.
