This is done by filling out a Notice of Motion and Supporting Affidavit form. Contact the clerk of the court to choose a time and date when the court could hear the motion. The clerk of the court will provide a hearing date and time. Put the date and time on the form.

The pathway asks you questions, gives you information to help answer them, and puts your answers into the required forms. The pathway is free to use. If you are asking the judge for a motion, you must fill out several forms. Complete a Form 14 (Notice of Motion) and a Form 14A (Affidavit). In Form 14, you will list the orders that you are requesting at the motion.
The motion must include a separate Notice of Motion which includes a brief summary of the nature of the motion, the deadline for filing a response, and if there is a hearing, the date, time, and location of the hearing.
