To apply for a passport you must present a certified copy and a plain copy of your birth certificate. If you were born in Arizona, you may obtain certified copies of AZ Birth Certificates from the Maricopa County Office of Vital Registration.
You can fill out the DS-11 form online or print out and fill out by hand. If you fill out the form electronically, at the end of the questionnaire you will need to print the form and bring it at the time of the interview.
If you need your passport sooner, the only Arizona passport office that processes same-day passports is the Western Passport Center, located at 7373 East Rosewood Street in Tucson. Appointments are required to use this service.
Thousands of Post Offices accept first-time passport applications for the U.S. Department of State. Most of those locations can also take your passport photo.
