To remove a photo from a collection as well as from the catalog, select the photo and press Ctrl+Alt+Shift+Delete (Windows) or Command+Option+Shift+Delete (Mac OS). See Remove photos from a collection. Removes photos from the catalog but doesnt send them to the Recycle Bin (Windows) or Trash (Mac OS).
Thankfully Lightroom has a few ways of helping out in this case. This usually happens when a file is moved outside of Lightroom after it was imported, or during the import a copy was used instead of add. Search the all photos collection for one of the images then right-click and show in Library folder to see where LR things the file should be.
One reason your photos go missing is because the external hard drive that Lightroom is referencing is not connected to your computer. The easiest way to check if you have a missing hard drive is to look at the Folders Tab in the Library Module.
