Grossly, early reperfusion results in necrosis of the inner third of the ventricular wall extending toward the mid-myocardium, whereas late reperfusion or permanent occlusion causes transmural fibrosis and a thinner ventricular wall.
In conclusion, in patients with AMICS, cause of death was mainly primary heart failure followed by neurologic injury and MOF. Median time from first medical contact to death was only 13 hours in patients dying from cardiac causes. The risk of dying of neurologic injury was low in patients without OHCA.
Symptoms Chest pain that may feel like pressure, tightness, pain, squeezing or aching. Pain or discomfort that spreads to the shoulder, arm, back, neck, jaw, teeth or sometimes the upper belly. Cold sweat. Fatigue. Heartburn or indigestion. Lightheadedness or sudden dizziness. Nausea. Shortness of breath.
The mortality rate of an inferior wall MI is less than 10 percent. However, several complicating factors can lead to increased mortality, including right ventricular infarction, hypotension, bradycardia, heart block, and cardiogenic shock.
The ECG findings of an acute anterior myocardial infarction wall include: ST segment elevation in the anterior leads (V3 and V4) at the J point and sometimes in the septal or lateral leads, depending on the extent of the MI. This ST segment elevation is concave downward and frequently overwhelms the T wave.
Primary complications of AMI include the following: New-onset mitral regurgitation. Ventricular septal rupture. Left ventricular aneurysm. Arrhythmias. Emboli.
These complications include arrhythmias and heart block (due to injured or ischemic conduction system tissue), and hypotension and congestive heart failure (due to ischemic or injured muscle tissue, resulting in abnormal filling {diastolic dysfunction} or abnormal emptying {systolic dysfunction}).
Evidence-Based Answer The most useful element for ruling in acute MI is chest pain with radiation to both arms, followed by radiation to the right arm.
