If the patient is being discharged to a rehab facility or nursing home, effective transition planning should do the following: ensure continuity of care. clarify the current state of the patientʼs health and capabilities. review medications. help you select the facility to which the person you care for is to be released.
When you leave a hospital after treatment, you go through a process called hospital discharge. A hospital will discharge you when you no longer need to receive inpatient care and can go home. Or, a hospital will discharge you to send you to another type of facility.
Online resources and literature suggest the following critical items for a discharge planning checklist: Arrange for caregivers and care location. Inform yourself about your condition. Review your medications and health supplements list with the hospital staff.
Discharge planning involves taking into account things like: follow-up tests and appointments. whether you live alone. whether someone can help you when you go home. your mobility. equipment needed for your recovery. wound care, if needed. medicines, especially if you need multiple medications. dietary needs.
The patients have to be able to recite the answers to the Five Ds of Discharge: Diagnosis, Drugs, Doctor, Directions and Diet.
Doctors also may discharge patients who: are uncooperative; do not follow medical advice; do not keep appointments; do not pay their bill; or are disruptive or unpleasant to the staff.
The discharge planning process involves an interprofessional team approach. Physicians are responsible for deciding the patient is safe for discharge, creating the discharge plan in conjunction with the rest of the team, and communicating instructions to the discharge nurse or designated discharge personnel.
