Narrator: Hi, and welcome to Dialed In: Helping Veterans Take Control of Their Health Care. Im Beth Doyle, a Veteran of the U.S. Air Force and an education specialist with the VA National Center for Ethics in Health Care. As a VA patient, you have the right to make your own decisions about your health care. You can accept or refuse any treatment or procedure, no matter what the outcome might be. But what if youre too ill to decide about your treatment? Who would you want to make those decisions for you? Fortunately, VA has a form you can fill out that covers that situation. Its called an advance directive. An advance directive tells your doctors, your loved ones and anyone else you care to name what your preferences are for health care, in case you cant make those decisions for yourself. Lucy Potter is an ethics policy specialist with the National Center for Ethics in Health Care and a licensed social worker who has helped Veterans complete advance directives. Potter: In the future

The two most common advance directives for health care are the living will and the durable power of attorney for health care. Living will: A living will is a legal document that tells doctors how you want to be treated if you cannot make your own decisions about emergency treatment.
The law clearly bars doctors from giving unwanted treatments. Even when the written directive is unclear, if a physician ignores an agents attempts to enforce the patients preferences, there are grounds for a lawsuit.
If you change or cancel your advance directive, be sure to let your health care team, loved ones, and your health care proxy (agent) know and also replace any advance directive copies you may have given them with the new one.
On the patients part, lack of knowledge, fear of burdening family, and a desire to have the physician initiate the discussion are common barriers.
Advance directives have limitations. For example, an older adult may not fully understand treatment options or recognize the consequences of certain choices in the future. Sometimes, people change their minds after expressing advance directives and forget to inform others.
Common Challenges With Advance Directives ing to Dr. Mirarchi, one common challenge with advance directives is something he refers to as medical stranger danger. Advance directives are documents often written with the help of attorneys and then interpreted by health care professionals, explains Dr.
A basic problem with creating a very specific living will is that hardly anyone can anticipate, years ahead of time, what treatments and interventions they will want or not want in circumstances they have never faced and have little experience of.
Advance directiveA written document (form) that tells what a person wants or doesnt want if he/she in the future cant make his/her wishes known about medical treatment. Artificial nutrition and hydrationWhen food and water are fed to a person through a tube.
