Definition: The Problem Record contains all details of a Problem, documenting the history of the Problem from detection to closure. A Problem is defined as a cause of one or more incidents; the cause is not usually known at the time a Problem Record is created.
What is a problem and how does it differ from an incident? As ITIL defines it, a problem is a cause or potential cause of one or more incidents. And an incident is a single unplanned event that causes a service disruption.
A request to change may be raised to modify an IT service in order to resolve a problem.An incident can lead to a problem then to change or directly from incident to change.
The following is a suggested list for when to open a problem record: Any incident that is assigned a Priority 1 or where a major incident has been declared. Multiple incidents showing the same symptoms. Validated alarms from monitoring devices that are deemed high impact.
When you create a problem record from an incident, Service Manager automatically associates the incident record to the problem record. Every time someone associates another incident to this problem, Service Manager updates the incident count for the problem.
The NIST incident response lifecycle breaks incident response down into four main phases: Preparation; Detection and Analysis; Containment, Eradication, and Recovery; and Post-Event Activity.
What is a Known Error? A known error is a problem that is successfully diagnosed and either a work-around or a permanent resolution has been identified. Known errors should be documented in the knowledge base as articles so that a resolution is captured and shared across the organization and the user community.
