We want this to register bodyweight. We click Add Tests, this opens up the Test Collection. The Bodyweight test is here. I also want to include a question, so I click on Add Information, which opens up the Information Collection. I want to add a field where the athletes can write a little bit about how they feel. I dont have that item in my collection but I can create it on the fly right here. Ill use the Plain text option. Thats it, now I have a form ready to send it out.
