Certified copies of name change judgement entries can be requested in person in Room 123 at the Probate Court, or by a written request through the mail (the request must include the case number, and names before and after the change).
The application, form, or resume title is used to identify the person completing the form. Titles on applications and documents can help determine your level of authority. For example, you may use a title like Dr. for an application to apply for positions as a lecturer or librarian.
To do this, search by name in The Gazette where all enrolled changes of name by deed poll have been announced since 1914. If you find an entry in The Gazette go to Step 2; if you do not find an entry we will not have a record of the change and you may need to speak to a solicitor if you require legal proof of identity.
The best way to access name change records is to contact the Judicial Archivist, Elizabeth Bouvier, at (617) 557-1082.
You can always find the form action by looking at the action field; for example: document. getElementById(myForm). action .
Editing the Form Name To edit the name of your form, navigate to the Name and Langauge section of the Form Properties and input text, numbers, or special characters into the Name box and save the changes to your form using the blue Save button in the top right-hand corner of the Form Builder.

Have the clerk schedule a hearing for you at least six (6) weeks in the future. The clerk should then return your certified copies.
