A release of information is a document that gives a consumer the opportunity to decide what material they want released from their medical file, who they want it delivered to, how long the data can be issued, and under what statutes and guidelines it is released.

How to send medical records to a new doctor (or get a copy for yourself) Talk to the new provider to determine what type of records they need. Visit or call your current physician. Submit the records request. Wait for the transfer to complete. Keep the records safe.
Physicians will require a patient to sign a records release form to transfer records. If you have followed the requirements outlined in the Health Safety Code and the physician has not complied with your request, you may file a complaint with the Medical Board.
Obtain written consent from the patient or legal representative. Confirm who the information should be released to, how to send it, and what information should be included or excluded. Validate that the requestor has the legal right to the information, especially when treating couples or parents of minor patients.
A proper and compliant medical records release form should include the following essential elements: Patient Information. Purpose of Request. Dates of Service. Recipient Information. Valid Authorization Signature. Date of Signature. Restrictions or Limitations. Revocation Clause.
As long as you requested your medical records in writing, to be sent directly to you (and not to anyone else, like your new doctor), the physician is required to send you a copy within specified time limits. If you are having difficulty getting your records, you can file a complaint with the Medical Board.
