The insertion point for lumbar puncture (LP) is typically in the L3-L4 interspace. To locate this space, find the highest point of the iliac crest and draw an imaginary line between the two posterior superior iliac crests. This line usually passes through the L3-L4 interspace. If you feel the spinous process of L3, move down slightly to locate the gap closest to the vertical line, which is the L3-L4 space. Alternatively, you can choose a higher space, such as L2-L3. Mark the spot with the opposite end of a sterile needle or use a finger from the opposite hand. Perform the procedure while sitting on a chair next to the patient's bed, which can be raised for comfort.
