G0101 Pelvic and Breast Exam Medicare developed two HCPCS codes for screening services for women, without certain frequency time limits. G0101(screening breast and pelvic exam) and Q0091 (obtaining a screening pap smear) may each be billed every two years for low risk patient and every year for high risk patients.
Separate reimbursement is not allowed for HCPCS code Q0091.
Coding for a Pap Smear Points to Note The CPT codes for cytopathology screening of cervical or vaginal smears are: 88141-88155, 88164-88167, 88174-88175, P3000, P3001, G0123-G0124, and G0141, G0143-G0148 are. The code submitted should reflect the service provided.
Cervical cancer testing (screening) should begin at age 25. Those aged 25 to 65 should have a primary HPV test* every 5 years. If primary HPV testing is not available, screening may be done with either a co-test that combines an HPV test with a Papanicolaou (Pap) test every 5 years or a Pap test alone every 3 years.
Pap test. A speculum is inserted into the vagina to widen it. Then, a brush is inserted into the vagina to collect cells from the cervix. The cells are checked under a microscope for signs of disease.
The new test for HPV can identify women who could be at risk of cervical cancer earlier than the Pap test could. Women aged 25 to 74 years of age should have a cervical screening test two years after their last Pap test. Subsequently, you will only need to have the test every five years if your results are normal.
Repeat Pap testing in one year This is generally the preferred option for people age 21 to 24. HPV testing This is also an option. If the HPV test is negative, you can return to your regular screening schedule. If the HPV test is positive, you should repeat the Pap test in one year.

Avoid intercourse, douching, or using any vaginal medicines or spermicidal foams, creams or jellies for two days before having a Pap smear, as these may wash away or obscure abnormal cells. Try not to schedule a Pap smear during your menstrual period. Its best to avoid this time of your cycle, if possible.
Pap screening may end at age 65 if the Pap history is unremarkable and the patient is low risk. low risk women 30 and above may go every 3 years if Pap only; or 5 years if cotesting. Risk factors that should be considered are HPV infection, STDs, HIV, smoking, new sexual partners, previous SIL or immunosuppression.
