A successful ACLR has traditionally been defined as a return to normal symmetric function based on multiple clinical measures, including muscular power and endurance, functional movement, and proprioception of the involved extremity.
Complete ACL tears have a much less positive outcome without surgery. After a complete ACL tear, some patients are unable to participate in cutting- or pivoting-type sports, while others have instability during even normal activities, such as walking.
Most people who tear their ACL feel pain and a pop in their knee when the injury happens. Their knee usually gets swollen soon after the injury. After the swelling goes down, someone with an ACL tear usually can walk. But the knee may feel unstable and can give way and make the person stumble or fall.
The IKDC Subjective Knee Form is the recommended knee-related outcome measure for ACL injury and treatment. Measurement of the PASS is valuable in the assessment of outcome of ACL injury and treatment.
In most cases, it is not possible to repair or reattach a torn ACL. ACL surgery usually involves a complete rebuilding of the ligament. This procedure, called ACL reconstruction, is the current standard of care for surgically treating a torn ACL.
ACL injuries can either be complete or partial. While complete ACL tears almost always require surgery, partial ACL tears may be treated effectively with nonsurgical methods. ACL tears are graded by severity and are called sprains (a sprain is a stretch or tear in a ligament).
Ideally, an outcome measure for patients with ACL injury would measure post-operative pain, mobility, quality of life, and return to functional activity (4,5).
Most people recover from an ACL tear within six to nine months.
Dont work your quadriceps early on because this can stretch the ACL graft. Stationery bike riding or lightweight leg presses are recommended during the first three months after surgery. These exercises strengthen the quadriceps while using the hamstrings to protect the ACL graft. Dont swim or run for five months.
The goal of this test is to jump as far as possible on each leg, aiming to stick the landing. The performance goal is to have the recently injured limb within 10% of the uninjured limb. Poor performance on this hop test (10% difference between limbs) can identify those at risk of re-injury.
