You fill out a form by filling in the blanks. Fill out can be used to mean youve completed something requiring your input. Fill in can be used that way, but less commonly. Filling in usually means adding something to an individual empty place.
Similarly, you can fill out a report. It might involve filling in fields, writing summaries, etc, but the overall act could be called filling it out. Filling it in has specific connotations of the act of filling in boxes, etc.
Fill in and fill out are both acceptable. Fill in is preferred in the UK and fill out is preferred in the US, but both are still correct in both countries (although fill in sounds less natural to American ears as the preference for fill out compared to fill in is more extreme there).
