Between the fibers of the ACL there can be fat or fluid and therefore the ACL is not completely black on PDWI images. Primary signs of ACL rupture are increased signal on MRI, fiber discontinuity, abnormal orientation and undetectable fibers [11].
Injured ligaments on MRI may appear disrupted, thickened, heterogeneous, or at tenuated in signal intensity, and may be ab normal in contour. Fluidsensitive sequences are often helpful in detecting injury.
MRI Anatomy of the Cruciate Ligaments. On sagittal T2-weighted images, the normal ACL appears as a straight band parallel to the intercondylar roof (Figure 1A). It fans out as it descends to its broad-based anterior tibial attachment. These collagen fibers of the ACL are organized into 2 principal bundles.
Most people who tear their ACL feel pain and a "pop" in their knee when the injury happens. Their knee usually gets swollen soon after the injury. After the swelling goes down, someone with an ACL tear usually can walk. But the knee may feel unstable and can "give way" and make the person stumble or fall.
Surgical treatment is recommended for individuals with a grade 3 or complete ACL tear. Surgical options may vary based on the type of ACL injury, whether the patient has open or closed growth plates, and the type.
Swelling often occurs immediately after an ACL injury. In some cases, swelling may develop up to 24 hours later and may last up to a week. A person who experiences an acute anterior cruciate ligament (ACL) tear of the knee is likely to report some combination of the following knee symptoms: Initial sharp pain.
