The University of Toronto has a way for students to have their name and/or gender changed on their academic records and ACORN. Go to your Registrars Office and request the Change of Name or Gender Request Form. You are able to change your name and/or your gender without legal documentation.
To appear on the diploma, a legal name change must be submitted to the Registrars Office, 1113 Murphy Hall, by the last day of the degree-expected term. Submit a Legal Name Change or Correction form. Documentation supporting the name change is required. There is no fee for this service.
The Florida Change of Name Act is found under Title VI, Chapter 68.07. Under this act, the general way to change a name is to file a petition with the court. However, this law also allows for different name change processes depending on the reason for the name change and the age of the requestor.
Generally speaking, names on academic diplomas arent changed unless a spelling or administrative error occurs. Just like your birth certificate, the university certificate is considered a matter of fact. Put simply, if the document was correct at the time it was issued, it wont be updated with your new name.
To change your legal name on your university record, you must complete a Name Change form (PDF) and provide acceptable supporting documentation. Completed Name Change forms, along with supporting documentation, can be submitted online through the Contact Form or in-person at the Student Service Centre.
Go to your local DHSMV office with proof of your legal name change, your current license or ID, proof of your Social Security number, proof of Florida residency, proof of citizenship (birth certificate or passport will work), and marriage certificate, if applicable. It will cost $25 to complete this process.
