The form must be signed by a physician, registered nurse, registered nurse in the extended class, or registered practical nurse. In ance with Ontario laws, the DNR form instructs paramedics and firefighters not to administer CPR (including use of breathing tubes and resuscitation drugs).
A licensed California physician must sign the DNR order, confirming the patients medical condition. The DNR order must use clear, unambiguous language specifying the individuals wishes. The DNR order should be readily accessible to healthcare providers and family members.
In the state of California, any resident over the age of 18 and who is mentally competent can complete an advanced directive. Two qualified adult witnesses or a notary public are required to sign off on the document.
The requirements for creating a DNR vary from state to state. In California, a DNR order must include the patients name and the date of the order, and must be signed by you (or your healthcare agent) and your physician. The DNR will be entered into your medical records.
A person who does not want to be resuscitated in such circumstances can give written instructions to that effect on a DNR order form which complies with the requirements of the California Probate Code. Usually persons who want DNR order forms are patients not likely to benefit from resuscitation efforts.
The DNR Form Ontario requires the patients name, date of birth, address, telephone number, and signature. Additionally, it requires the patient to specify their medical wishes regarding cardiopulmonary resuscitation (CPR), medical treatments, and other end-of-life decisions.
The purpose of the DNR-C form is to provide direction to emergency responders (i.e., paramedics and firefighters) in the community regarding a patients resuscitation status.
In summary, the minimum requirement in order for a DNR form to be valid is the valid signature and date of the patient, or of their legally recognized healthcare agent in the event that they are incapacitated or otherwise unable to express their wishes.
