An advance health care directive lets your physician, family, and friends know your health care preferences, including the types of special treatment you want or dont want at the end of life, your desire for diagnostic testing, surgical procedures, cardiopulmonary resuscitation and organ donation.
WILL I STILL BE TREATED IF I DONT MAKE AN ADVANCE DIRECTIVE? Absolutely. You will still get medical treatment. We just want you to know that if you become too sick to make decisions, someone else will have to make them for you.
The form must be signed by two qualified witnesses or acknowledged before a notary public. Basic barriers include thinking that an advance directive isnt needed, not wanting to think about death or serious illness, not wanting to burden people, not knowing enough about advance directives and health care choices, needing help to fill out the forms, and lack of time with the doctor to discuss the matter.
You must sign this form in front of witnesses before it can be used or given to your doctor. PART 4: Make your Advance Directive legally valid. To make valid in California, your Advance Directive must be signed by two witnesses, OR acknowledged before a Notary Public.
