Management of ACL injuries can vary based on severity and the treating healthcare provider. Generally speaking, Grade 1 injuries are managed without surgery, Grade 2 injuries can be managed with or without surgery, and Grade 3 injuries with surgery. There are exceptions, but this is what is often observed.
Partial ACL Tear (Grade 2): The ACL stretches and becomes loose. Doctors often refer to this type of ACL injury as a partial tear of the ligament. This type of ACL tear is not common. Complete ACL Tear/Complete Ligament Tear (Grade 3): The ACL is completely torn split into two pieces making the knee unstable.
The grade of ACL rupture is classified based on the degree of anterior tibial translation in mm. Grade 1 (3-5 mm), grade 2 (5-10 mm), and grade 3 ( 10 mm translation). However, the injured side should always be compared with the good side.
Grade one: The injury stretched your ACL enough to damage it, but its still in one piece and holding your knee bones together. Grade two: The injury stretched your ACL so much that it was partially torn and loosened. Grade three: A complete tear your ACL is in two pieces.
Readiness for sport was based on criteria for the ACL-RSI scale score (low threshold, 56 points or greater; high threshold, 75 points or greater) and hop tests (90% or greater limb symmetry).
ACL-RSI Scale This 12-item scale is designed to measure psychological readiness to return to sport after ACL injury or reconstruction surgery. It includes 3 domains: emotions, confidence, and risk appraisal. Scores for each item are summed and averaged for a total score between 0 and 100.
What You Need to Know. ACL injuries are more common among girls than boys. Treatment for ACL injuries in children requires special consideration because of the location of growth plates. ACL injuries are classified by grades 1, 2, and 3.
The procedure is performed under general anesthesia. Your surgeon will make two small cuts about 1/4-inch-long around your knee. An arthroscope, a tube with a small video camera on the end is inserted through one incision to see the inside of the knee joint.
