Learn where to find PDF templates in the web browser application. Click the Create Template button. Name the template and click Create. The template will be created. Click on template name to edit properties.
In the Templates panel, click the + icon next to Templates and select PDF Template. Specify a name for the template in the New PDF Template dialog. Click Done.
To use your existing form as a template for the next time you need to create a similar document, simply open your original file and choose Save As. Rename your new file and make any necessary changes you need to prepare it as your next fillable form.
How to create fillable PDF files: Open Acrobat: Click on the Tools tab and select Prepare Form. Select a file or scan a document: Acrobat will automatically analyze your document and add form fields. Add new form fields: Use the top toolbar and adjust the layout using tools in the right pane. Save your fillable PDF:

Creating a template based on an existing PDF document After opening the document in PDF Annotator, we choose File, Save as Template from the menu. We can fill in a name for the template, or just leave the default.
Create a library template To create a library template, click the Home tab, then select Create a reusable template. The Create Library Template configuration page loads. Drag and drop your files into the Files section, or click the Add Files link at the upper-right corner of the Files field to browse your system.
If you select the button and then look in the properties there is an option called tooltip. This will provide the functionality that you need. Just enter the text you need and voila!
