In evidence-based medicine, likelihood ratios are used for assessing the value of performing a diagnostic test. They use the sensitivity and specificity of the test to determine whether a test result usefully changes the probability that a condition (such as a disease state) exists.
A test with an LR of 1.0 indicates that it does not change the probability of disease. The higher above 1 the LR is, the better it rules in disease (an LR greater than 10 is considered good). Conversely, the lower the LR is below 1, the better the test result rules out disease (an LR less than 0.1 is considered good).
The likelihood ratio, then, is a way of summarizing the DNA evidence. If the LR is 1,000, the probability that the profiles are the same is 1,000 times as great if the samples came from the same person as it is if they came from different persons.
Patient-oriented and disease-oriented outcomes may agree or disagree. For example, the evidence that diuretics effectively decrease blood pressure in hypertensive patients (a disease-oriented outcome) agrees with the evidence that they reduce mortality from stroke (a patient-oriented outcome).
This is called patient-oriented evidence, or POEs. Often, the outcome studied isnt as helpful, something called disease-oriented evidence, or DOEs. POEs are things like mortality, morbidity, and quality of life. Studies that look at POEs are asking questions like Do people live longer, healthier or happier?
