On the Design tab, in the Tools group, click Add Existing Fields....Add a field to a form or report by using the Field List pane Double-click the field. Drag the field from the Field List pane to the form or report. Hold the CTRL key and click several fields, and then drag them all to the form or report at the same time.
Rename a Document Right-click the report name or click the three-dot More menu at the end of the report row. Select Properties. Edit the Title. Click Save.

Headers include: Title of Document. Sub-Title or Chapter or Section....Footers include: Name of Author (very important) Date of Publication. File Name (optional) Version Number (optional) Page Number.
In the Navigation Pane, right-click the form or report and then click Design View or Layout View on the shortcut menu. On the Design tab, in the Header/Footer group, click Title. A new label is added to the form or report header, and the form or report name is displayed as the title.
To modify the Word report layout To make basic layout changes, such as adding or removing tables and cells, formatting text, or changing margins, use the editing features of Word. To add fields from the report dataset to the layout, use the custom XML part for the report to add content controls that map to the fields.
Click File, and then look for Author under Related People on the right. Right-click the author name, and then click Edit Property. Type a new name in the Edit person dialog box.
Add or edit a report title On the Design tab, in the Header/Footer group, click Title. ... When the label is created, the text in the label is selected for you so that if you want to change the text, you can just begin typing the title you want. Press ENTER when you have finished.
