If a students waiver is denied, an appeals form may be requested from the Insurance Services Office for reconsideration. Appeals forms must be completed and returned with all requested supporting documentation by applicable deadlines.
Your UC SHIP Insurance ends on the last day of the quarter within which you graduated. (With the exception of Spring Quarter, insurance coverage extends through Summer Quarter) Graduated students will have a (1) quarter grace period to receive services at Student Health Service.
To waive out of UC SHIP, you must submit a request on-line during the open waiver periods and by the posted deadlines as shown below. If approved, the waiver is in effect for the current term and the remainder of the academic year. A new waiver request must be submitted at the beginning of each academic year.
