Included in AFPs list are poliomyelitis (polio), transverse myelitis, GuillainBarr syndrome, enteroviral encephalopathy, traumatic neuritis, Reyes syndrome, etc. An AFP surveillance programme is conducted to increase case yield of poliomyelitis.
Acute flaccid paralysis (AFP) is defined as a sudden onset of paralysis/weakness in any part of the body of a child less than 15 years of age. Each case of AFP acts as a signal to the polio surveillance system where polio is circulating and who it is likely to affect.
Stool adequacy: This refers to the proportion of AFP cases with two adequate stool samples ( 8 g) collected in 24 h apart and within 14 days after onset of paralysis, transported on ice/frozen packs and received in a WHO-accredited laboratory in a good condition. The recommended target is 80% [18, 19].
Case detection and reporting Nationwide AFP surveillance is the gold standard for detecting cases of polio, using the following recommended standard case definition: Any child under 15 years of age with AFP (including Guillain-Barr syndrome) or any person of any age with paralytic illness if polio is suspected.
Clinical specimens for virological investigation have to be collected. A follow-up reporting form is sent to CNE to determine paralysis evolution after 60 days from paralysis onset, and to clarify the final diagnosis of the case. Cases are classified as confirmed, discarded or compatible with polio [12,13] (Figure 3).
Acute flaccid paralysis (AFP) describes a group of conditions with set symptoms. AFP is identified by the sudden weakening or loss of muscle tone in children less than 15 years old, without a clear cause. Viruses or conditions, such as poliovirus, West Nile virus, or Guillain-Barr syndrome, may cause AFP.
AFP surveillance index is calculated by multiplying the nonpolio AFP rate per 100 000 population by the proportion of cases with stool specimens.
Stool Collection and Storage procedures a. Collect two adults thumb-sized stool specimens (at least 24 hours apart) from each AFP case within 14 days from paralysis onset. b. For watery stool specimen, collect at least 5 ml of the stool specimen.
