The body of the prescription, or inscription, contains the name and amount or strength of the drug to be dispensed or the name and strength of each ingredient to be compounded.
The following are minimum requirements for a legal prescription: Written or printed legibly in indelible ink. State a valid date. Signed in ink by the prescriber. State the address of the prescriber and an indication of the type of prescriber. State name and address of the patient. Specify the age for children under 12 years.
Example: Paracetamol ii tds x 5/7 = meaning 2 tablets 3 times daily for 5 days.
Sig codes indicate medication dosages, measurements, dosage times or frequency, and routes of administration to a pharmacist and a pharmacy technician.
Sig is short for the Latin signetur. This means let it be labeled. You may see this on your prescription just before the directions. Sig tells the pharmacy what they should include on the drugs label. This ensures you will know how and when to take the medication.
The subscription is the instruction to the pharmacist, usually consisting of a short sentence such as: dispense 30 tablets. The signa or Sig is the instruction for the patient as to how to take the prescription, interpreted and transposed onto the prescription label by the pharmacist.
Signatura (Sig) The Sig should include information on how much drug to take, how to take it, and how often to take it. For example, if you would like your patient to take one 650 mg tablet of acetaminophen every six hours, you would write Take 1 tablet by mouth every six hours or, using abbreviations, 1 tab PO q6h.
Examples of SIGs include Take one tablet by mouth every four to six hours, or Take 2 tablets by mouth once daily on day one then 1 tablet once daily on days two through five.
